Jack H. Jacobs was born August 2nd, 1945, in Brooklyn, NY. He moved to New Jersey and attended high school Woodbridge. He went on to Rutgers University and earned a bachelors degree. He joined the ROTC and then entered the Army in 1966.

Jack H. Jacobs showed his gallantry and heroism saving the lives of 1 U.S. Advisor and 13 Allied soldiers. Through his effort the allied company was restored to an effective fighting unit and prevented defeat of the friendly forces by a strong and determined enemy. He put his life on the line for other allied soldiers.
